This looks like some sort of stove or agricultural wrench, looks really useful, the only marking is the PATT 2450 which isn't any help at all. How about a guess or two before using Google lens to identfy it?
The 5 sided wrench was the clue for me. I don't study hydrants much, but the 5 sided wrench is used as tamperproof bolthead on manhole covers around here...

I'm thinking the same thing.The five sided bolt is right on top of USA hydrants,at least most I've seen.

Yes, a hydrant wrench. Canadian hydrants (such as Terminal City Iron Works) also use the 5 sided operating nuts.
