Looks like a tool for adjusting the rocker arm clearance on an overhead valve engine. You loosen the lock nut with the wrench then turn the knob until you get the specified clearance between the rocker arm and the valve stem then tighten the lock nut. It has a provision for two different size nuts. Nice.
